St. Mary's Heart Success Program provides additional support between your regular cardiology care visits, with special focuses centered on education, monitoring, and day-to-day management of your symptoms.

Yes, a referral from your provider is typically required to participate. If you require a referral, a member of our Primary Care practice can help get you started with this process. Many of our providers offer same-day or next-day appointments, so that we can help get you the care and treatment that you need, fast.

The number of appointments, meetings, and sessions will vary based on your personal needs and treatment plan, but every member of our Heart Success program is expected to take part in regular check-ins with their care team and maintain ongoing communication to ensure that your tailored program is effective, working, and fits your lifestyle.

No. Joining the Heart Success program does not mean you no longer need to see your Cardiologist or engage in previously prescribed care. This program is meant to work alongside your cardiologist and care team to provide additional support.

By monitoring symptoms and addressing concerns early, the program can help reduce the risk of complications, however, it does not guarantee that you will never experience a cardiac event or episode. 

Through our program, patients learn how to manage symptoms, how to take their medications correctly, and how to make long-term lifestyle changes that will help support their heart health and increase their longevity.
